Puppy will lie down, pant for a couple minutes, then pee herself immediately after

I’m feeling pretty overwhelmed right now and could really use some advice. I have a 4-month-old puppy, Daisy, and she has this odd behavior where she'll lay down, start panting heavily for a few minutes, and then suddenly pee on herself and the floor. I'm kind of at my wit’s end trying to figure this out. It's not just when she's excited or after she's been playing—sometimes it happens when she seems completely calm and just lounging around. I’ve been diligent about taking her out regularly, so I’m confused about why this is happening. Does anyone have any idea what might be going on? Is this something normal for puppies or should I be worried? I want to make sure she's okay and maybe ease my own stress too. Thanks in advance for any tips or thoughts you might have!
