My cat won't stop getting parasites

I'm really hoping for some advice here because I'm at my wit's end. My cat, Oscar, has been battling what seems like a never-ending cycle of parasites, and I just don't know what to do anymore. We've been to the vet multiple times, and every time we think we've got it handled, they just keep coming back. It's been a mix of fleas and, more recently, worms. I try to keep everything clean and I use the treatments the vet recommends, but it feels like as soon as we're clear, the whole ordeal starts again. Oscar is scratching a lot, seems uncomfortable, and it's breaking my heart to see him like this. Plus, I'm worried it’s affecting his health in the long run.
