Do I need to take my dog to the vet?

I'm really worried about my dog, Max. He's usually super energetic and playful, but over the last couple of days, he's been really lethargic and just not himself. He hasn't eaten much, and when he does, he seems to have trouble keeping it down. Also, I've noticed him scratching a lot more than usual, and there's this weird rash on his belly. I'm freaking out a bit because I'm not sure if I'm overreacting or if this is something serious. I don't want to be the kind of person who rushes to the vet over every little thing, but I also don't want to ignore something that might be important. Can anyone share some advice or similar experiences? Is it time to call the vet, or should I wait it out a bit longer and see if he improves? Thanks in advance.
