Cat died in recovery from spay surgery - trying to understand why

I'm really struggling and could use some guidance from this amazing community. Yesterday, I took my 2-year-old cat, Luna, in for what I thought would be a routine spay surgery, but she tragically didn't make it through recovery. I'm absolutely devastated and can't stop replaying everything in my head. She seemed healthy and full of energy before the surgery, and I trusted the vet completely. They said complications were rare, but I never thought this would happen to Luna. I've been trying to understand what could have gone wrong, and if there was anything I missed or should have known. Did she have an underlying condition? Was it an anesthesia issue?
